Following detailed history and examination, a pelvic ultrasound is recommended as a 1st line simple investigations. Ultrasound allows the consultant to exclude abnormalities in the uterine wall and in the uterine cavity, since malformation, Polyps or fibroid may affect the outcome of the treatment. It also helps to assess the ovaries and observe its position in relation to the uterus. If the ovaries, are too close or above the uterus this may indicate the presence of adhesions due to endometriosis or previous infection. Vaginal ultrasound also allows observation of the number of follicles which contain eggs, an indication of ovarian reserves. Ovarian reserves are important to decide on best treatment options and allows fine tuning of medications during treatment cycles. Semen analysis is also important since 1:6 couples fertility problem is due to abnormal sperm count. Tubal test by Hycosy or HSG may be necessary to assess the tubes and the uterine cavity.
Aquascan or Saline Hysterography allows better visualization of the uterine cavity similar to Hysteroscopy. A normal cavity is necessary for proper implantation and continuation of the pregnancy.
