All new patients at The Fertility & Gynaecology Academy are required to complete our patient registration form(s) prior to your first consultation. This information is required by the HFEA and treatment cannot commence without it.

If you have a printer you can download the Female Patient Registration Form and/or Male Patient Registration Form. These form(s) must be printed, filled in, scanned and returned to info@fertility-academy.co.uk. Please note that if you are in a relationship we will require a form to be filled out for both partners.

Alternatively, you can complete the form(s) online which will send your details directly to us. We will process your data in accordance with our Privacy Policy.
